End-of-Life Issues

Our patient navigators, social workers, chaplains (pastoral care) and the physicians themselves work with patients and families to determine the most appropriate course of action when facing end-of-life issues. We do our utmost to make sure that each patient is well-informed and monitored as they transition from diagnosis and treatment to palliative or hospice care.

Hospice
Hospice care is provided throughout the service region by 21 local hospice programs. The Palliative Care committee addresses timely referrals to hospice to improve patient transitions along the continuum of care.
