My Medication List™ is a handy tool for keeping track of your medications all in one place in case you need that information for a hospital stay or other health care needs. This information can be very valuable to healthcare workers or pharmacists trying to care for you.

If you are scheduled to have a procedure performed at
St. Joseph's/Candler, we encourage you to fill out this form and bring it with you to your pre-operative visit or surgery.

Note: If you have the latest version of Internet Explorer, you will be able to fill in the information electronically and print out the form.  To save this information, please download the form to your personal computer.  This information is not maintained or saved on any external server and will be deleted when you exit the browser; therefore, please make sure you save it to your home computer for future updating.

If you use a browser that does not support it or have an earlier version of Internet Explorer, you may not be able to fill out the form electronically.  In this case, please use the print option on your tool bar and print the form manually, then simply fill out by hand.
