Many conditions previously requiring extended hospital stays are now being treated at home, where patients tend to be happier and more at ease. Home Health Care provides continuity of care in the transition from hospital to home, including medical treatments.
Our home care agencies are accredited by The Joint Commission and are Medicare- and Medicaid-certified. We are ranked in the "Top 500 Home Care Agencies" in the nation (HomeCare EliteTM 2007, 2008, 2009 and 2010) and participate in the Georgia Medical Care Foundation (GMCF) Quality Improvement Organizations (QIOs) activities. Our integrated team of thoroughly trained professionals includes:

We take a total-patient approach to treatment, which means that we develop specialized plans of care that are the most appropriate for each patient's individual needs and level of functioning. The goal of these individual plans is to reduce recovery time and optimize independence.

Services
To meet the various needs of home care patients, our treatment programs include a wide range of services, which range from occasional visits by home care staff to total clinical support 24 hours a day, seven days a week. Services are provided under the direction of the patient's physician.

Locations
Office hours are Monday through Friday, from 8 a.m. until 5 p.m.; however, our professionals are available 24 hours a day, seven days a week. We serve the following counties: Chatham, Effingham, Liberty, Bryan, Evans, Long, Brantley Camden Glynn, McIntosh and Wayne.
