SOURCE at St. Joseph's/Candler

Nationally recognized as a model program, SOURCE (Service Options Using Resources in Community Environments) monitors the health care of Medicaid and Medicare patients, many of whom are older with limited income. It strives to keep those with chronic conditions healthy and living independently while improving their health outcomes. SOURCE provides enrollees with a blend of medical care and preventive and supportive services, linking three vital health components:

  • Primary medical care
  • Community services
  • Case management

SOURCE Goals

  • Prevent the increase of disability and disease in chronically ill adults.
  • Eliminate fragmented service delivery using managed care principles, outcome-based case management and relief from programmatic constraints.
  • Increase the cost efficiency and value of Medicaid long-term care funds by reducing inappropriate emergency room use, multiple hospitalizations and nursing home placement caused by preventable medical complications and by promoting self-care and informal support when possible for individual members.
  • Reduce long-term institutional placement and increase options in the community for older and disabled Georgians.

Based on the success and program elements of this St. Joseph's/Candler program, our initiative has been implemented in several other Georgia cities.
