We help people with diabetes gain control over their disease by using the latest, most effective management skills. We provide diabetes education and recommendations according to the American Diabetes Association (ADA) Standards of Care in an outpatient setting on Candler Hospital's campus.

Our skilled, compassionate staff is up to date on the latest diabetes advancements and technology. Nurses practitioners, dietitians and pharmacists work closely with physicians, combining clinical expertise with genuine concern, to help people with diabetes and their families self-manage the disease.

Managing Diabetes for Improved Quality of Life

Certified diabetes educators help hospitalized patients with diabetes develop an individualized program, which includes medication review, blood sugar measurement and diet instruction. Upon discharge, patients are encouraged to continue in-depth diabetes education and attend our outpatient diabetes self-management program.
