Thank you for choosing St. Joseph's/Candler for your surgical needs. We know you have a lot on your mind the day of a procedure. To help make the process as seamless as possible, you can now fill out paperwork electronically prior to the day of your procedure. This will save you time as our staff will have your medical and medication history prior to your surgery date. We have two online forms: pre-registration and pre-screening.

The information is not required of every hospital patient. ONLY patients having procedures in the following areas should fill out the paperwork:

  • Day Surgery
  • Specialty Surgery Center
  • GI/Center for Digestive Diseases
