We recommend completing all necessary forms before your first visit to help you get started quickly and focus on your therapy session. This ensures we can begin your therapy promptly and keep everything running smoothly.

We also encourage involving your family and close friends in your rehabilitation journey. Their support can be a powerful motivator and a key component in your recovery process. Your therapy team welcomes family and friends to attend appointments, ask questions, and actively participate in your progress. If you'd like to invite someone to join you, please notify your therapist so we can create a collaborative and welcoming experience for everyone.

Patients under 18 must be accompanied by a legal guardian to the registration appointment.

How to register for outpatient therapy services

To begin your therapy journey, follow these steps:

1. Obtain a Physician’s Referral
   A physician’s order is required to start physical or occupational therapy. If you believe therapy can help your condition, discuss it with your doctor and request an order.
2. Complete Required Forms
   Download and complete the following forms at home:
   • Medical History Form
   • Rules and Guidelines

Bring these complete forms with you to make your first appointment to save time.

1. Schedule Your Appointment
   Once you have your physician’s order, contact your preferred St. Joseph's/Candler outpatient rehabilitation location to schedule your first session.
2. Confirm Insurance or Payment Options
   Verify your insurance coverage or inquire about payment plans if needed. Our team is here to assist with financial arrangements if you are uninsured.

3. Visit One of Our Convenient Locations
