St. Joseph’s/Candler’s Home Health Care serves homebound patients in more than 15 Coastal Georgia counties. Our treatment programs include an extensive range of services, which are provided under the direction of the patient’s physician. On-call nurses are available 24 hours a day, seven days a week. Our specialty services are as follows:

Infusion services: We provide patients with essential medications administered directly into the bloodstream, ensuring effective and timely treatment in the comfort of their own homes.

Who qualifies for home health?

Home health care is an essential solution for patients who need medical support but do not require full-time hospital care. To qualify for St. Joseph’s/Candler Home Health Care, patients must meet specific criteria:

A doctor’s order for home health services is required.

The patient must need skilled nursing, therapy or speech-language pathology services.

Additionally, patients must be considered homebound, meaning it is challenging to leave home without assistance.
