St. Joseph’s/Candler offers comprehensive diabetes management programs through our Center for Diabetes Management. With a physician referral, individuals with diabetes or those at high risk can join our counseling program, where they’ll receive personalized guidance and specialized support to navigate their journey.

Our programs are designed to enhance the quality of life for those with diabetes by promoting a deeper understanding of the disease and its management.
Year-long Diabetes Management Program
For patients that may require a comprehensive program to meet their needs, the Center for Diabetes Management St. Joseph’s/Candler offers a year-long program tailored to support patients newly or previously diagnosed with diabetes, whether or not they face complications. Following the American Diabetes Association’s recommended guidelines, this program provides expert care and education to help individuals effectively manage their condition and improve their quality of life.
With a physician's referral, patients gain access to a full range of services designed to meet national standards for diabetes care, ensuring they receive the highest level of support on their journey to better health. Our comprehensive services include the following:
- Education and other services provide thorough diabetes management that meets national standards.
- Our services are offered by registered nurses and licensed dietitians who are experienced, certified diabetes educators.
- Home glucose monitoring equipment and supplies are provided for effective diabetes management.
- Consistent lab work is conducted to monitor diabetes control.
- A foot exam is performed to check for any rising complications.
- A dilated retinal eye exam is conducted for your peace of mind.
- Patients will have quarterly follow-up visits at the Diabetes Management Center throughout the year with their case manager who is there to answer all concerns.
- A flu vaccine is offered to help prevent illness.
- Smoking cessation programs are available as needed to support overall health.
Family members' participation in these programs is strongly encouraged to give our patients the best support possible.
The American Diabetes Association annually certifies our outpatient Diabetes Management Program, which is also credentialed as a Diabetes Prevention Program by the Centers for Disease Control and Prevention. This designation is reserved for programs that have effectively delivered a quality, evidence-based program that meets all the standards of the CDC recognition.
Funding should never be considered when you are on the road to recovery and wellness. This is why our program is 100 percent covered for St. Joseph’s/Candler co-workers, Georgia Ports, Fuji Vegetable Oil, Chatham EMS, Colonial Oil, City of Savannah and Chatham County employees. Year-long Pre-Diabetes Management Program
The Pre-Diabetes Management Program is designed for individuals with prediabetes or those at risk of developing Type 2 diabetes. Participants receive personalized guidance on how to eat healthier, incorporate physical activity into their daily lives, manage stress, stay motivated and overcome obstacles that can stand in the way of lasting, healthy changes.
By focusing on prevention, we empower you to take control of your health before diabetes takes control of you.
This program's health benefits are life-changing. It can help you reduce the risk of diabetes and improve your overall well-being for years to come.
- CDC-approved curriculum with lessons, handouts and other resources to help you make healthy changes.
- A lifestyle coach specially trained to lead the program to help you learn new skills, encourage you to set and meet goals and keep you motivated. The coach will also facilitate discussions and help make the program fun and engaging.
- A support group of people with similar goals and challenges. Together, you can share ideas, celebrate successes and work to overcome obstacles.
- Candler Wellness Center membership throughout the duration of the program to promote holistic wellness and community.
- Biometrics and A1C at the beginning of the program, then once every six months.
A physician’s referral is also required for this program. Medical Nutrition Therapy at St. Joseph’s/Candler offers personalized, one-on-one sessions with a registered dietitian to help adults and children address specific medical concerns through nutrition. We provide complete support for benefits such as cholesterol reduction, sodium control, weight management, gastrointestinal disorders or managing renal diseases. Our expert dietitians provide tailored guidance to improve your health and well-being.
