Our programs, the Community Care Services Program (CCSP) and Service Options Using Resources in Community Environments (SOURCE) at the Georgia Infirmary, provide compassionate and comprehensive care to help individuals remain at home and avoid institutional placement. These case management services are part of the Elderly and Disabled Waiver Program (EDWP), administered by the Department of Community Health. Our highly trained and specialized teams provide expert care, making us one of the region's most trusted and top-rated services.
What are CCSP and SOURCE?
CCSP and SOURCE are specialized case management programs that offer an alternative to long-term institutionalization. They allow our elderly and disabled individuals to stay within their community with the support of coordinated care services. CCSP and SOURCE remain available to provide compassionate, tailored care for those under hospice care.
Enhanced case management services offer coordination with primary care physicians to manage chronic conditions like diabetes, congestive heart failure (CHF), or chronic obstructive pulmonary disease (COPD). This level of comprehensive oversight ensures that our patients receive the highest level of care, with access to personalized medical management.
Essential services available through the programs include:
- Adult Day Health: Nursing services, recreational activities, and lunch are provided in a day center.
- Alternative Learning Services: Residential support in a personal care home for individuals requiring 24-hour supervision.
- Case Management Services: Assessments, care coordination, monitoring, education, and resource management.
- Emergency Response System: Call button service for immediate assistance in emergencies.
- Home-Delivered Meals: Nutritious meals are delivered directly to the house.
- Home Health Services: Nursing and therapy services are provided in the home.
- Personal Support Services: Assistance with daily activities like bathing, dressing, light housekeeping, meal preparation, shopping, and respite care.
- Structured Family Caregiving: Support, education, and oversight for family caregivers residing with the member.

If you qualify, we will schedule a convenient in-home nursing assessment to ensure your loved one receives the personalized care they deserve. After the evaluation, we will handle the application submission to the Department of Community Health on your behalf.
