Our dedicated team of nurse navigators, social workers, chaplains (pastoral care), and physicians work together to guide families in making well-informed, compassionate decisions about care. From the moment a patient transitions from active treatment to palliative care or hospice, we prioritize personalized care that aligns with the patient's values, beliefs and wishes. Our goal is to ensure that every patient is supported and treated with dignity during this deeply personal stage of life.
Palliative Care
Our palliative care services focus on enhancing the quality of life for patients with serious or life-limiting illnesses. Palliative care is not always end-of-life care. This specialized care addresses physical, emotional and spiritual needs, with an emphasis on symptom management, pain relief and emotional support. The palliative care team works closely with patients and their families to create individualized care plans that honor their preferences and goals
Hospice Care
Hospice care at St. Joseph's/Candler is designed to provide compassionate support for patients in their final stages of life. Partnering with local hospice programs throughout our region ensures seamless patient and family transitions. This care extends beyond the patient, with bereavement support and counseling services available to families during and after their journey.
We work with community-wide organizations such as The Steward Center for Palliative Care.
Timely Referrals and Continuity of Care
One essential aspect of our end-of-life care approach is ensuring timely and appropriate referrals to hospice care. Our Palliative Care team collaborates with hospice providers to improve patient transitions and maintain continuity of care. By addressing these transitions promptly, we help patients avoid unnecessary hospitalizations and focus on comfort and quality of life at home or in hospice settings.
Support for Families
Families play a crucial role in caring for their loved ones, and we are here to support them every step of the way. Through resources tailored to their needs, we empower families to make confident decisions and provide meaningful support during this time. Our chaplains and social workers are available to offer guidance, prayer, and emotional support, while our nurse navigators ensure families have access to the resources and information they need.
